Use this form to apply for additional units in a Fund or Funds in which your clients are already invested. Make sure the form is signed by the appropriate parties. Appointment of nominated representative form - applies to BlackRock's Investment Funds range of products. It allows your clients to nominate a person to act as their authorised representative/ agent and operate aspects of their account with BlackRock. Please ensure that you follow these instructions when making a full or partial switch from one investment portfolio to another. Investors are not obliged to quote their Tax File Number. However, if they are an Australian Resident for tax purposes and do not quote their Tax File Number, we are obliged to deduct tax at the highest marginal rate, plus the Medicare Levy, from income distributions.
